However if the condition is not managed in the early stages, your health could deteriorate and that is when you could start to feel sick. Knowing where to get support or where to find out more information about your condition can be confusing.

Early Intervention in Chronic Disease Services may be accessed directly by people with a chronic disease, their family members and care givers.
Referrals from your GP or other health professional may be emailed or faxed to the above contacts.
